Job summary

Circadia Health is seeking an on-site Nurse Practitioner in Detroit, MI to evaluate residents, act on predictive health signals, and mentor facility teams in using Circadia to intervene earlier. You will blend direct patient care with technology-driven bedside integration through hands-on training and live demonstrations.

Applicants should be licensed as NPs in Michigan with at least two years of experience serving complex adult populations, and be comfortable traveling locally.

Qualifications

Licensed Nurse Practitioner in Michigan (ANCC-certified or equivalent).
Eligible for Medicare participation; company bills on your behalf.
Minimum 2 years NP experience with complex adult populations.
Valid MI driver’s license; ability to travel locally.
English required; startup environment experience preferred.

Responsibilities

Direct patient assessments and adjust care plans.
Train staff to integrate Circadia into clinical workflows.
Interpret AI signals to prioritize high-risk residents and initiate interventions.
Complete documentation on the same day.
Perform high-volume rounds (30+ visits/day) across SNFs where applicable.
Provide feedback to product/ops to improve workflows.
