Rocky Mountain Health Care Services (RMHCS) seeks a Nurse Practitioner to join the PACE IDT, delivering wraparound care for older adults in the Pikes Peak region. You will collaborate with PCPs, nurses, social workers, rehab therapists, and others to support patients aging in place with dignity and equity.

You’ll build meaningful connections with participants and colleagues, contributing to high-integrity, person-centered care in a deeply relational role.”

Qualifications

  • Current NP licensure (AGNP, FNP, GNP) and prescriptive authority in Colorado.
  • Experience in geriatrics, family/internal medicine; interest in palliative care a plus.

Rocky Mountain Health Care Services (RMHCS) is dedicated to improving lives, optimizing wellness, and promoting independence for participants in the Pikes Peak region through our Program of All-Inclusive Care for the Elderly (PACE). We are a nonprofit serving older adults who live with serious illness, frailty, and socioeconomic hardship. About the Role

We are seeking a curious and collaborative Nurse Practitioner to join our PACE IDT as part of a robust interprofessional care model. You’ll work hand-in-hand with PCP partners, nurses, social workers, rehab therapists, behavioral health specialists, dietitians, recreation therapists, chaplains, and others to deliver wraparound care that engages older adults as they age in place, on their own terms.
