HopeWest in Grand Junction, CO is seeking a PACE Nurse Practitioner to provide primary care, case management, and care coordination for a panel of participants, under state and federal guidelines. The NP will collaborate with physicians and specialists to develop the plan of care, focus on prevention, wellness, and management of acute and chronic conditions, and ensure timely documentation in the EMR.
A current Colorado APN license, DEA license, and MS Office/EMR skills are required.
Hiring wage range: $54.25–$60.00/hr
The PACE Nurse Practitioner (NP) is an advanced practice nurse, providing clinical oversight and medical care in accordance with state and federal practice laws. The NP collaborates internally to establish realistic goals and a thorough, up‑to‑date participant plan of care (POC). The NP serves as the primary care provider for an assigned panel of participants, providing primary health care and case management, focusing on disease prevention, wellness, and management of minor, chronic, and acute illnesses. The NP collaborates externally with attending physicians, specialists, and other providers as applicable for the purpose of coordination of care to effectively meet the POC of participants.
