We are a nonprofit Program of All-Inclusive Care for the Elderly (PACE) serving older adults who live with serious illness, frailty, and socioeconomic hardship. We are seeking a curious and collaborative Nurse Practitioner to join our PACE IDT as part of a robust interprofessional care model. You’ll work hand-in-hand with PCP partners, nurses, social workers, rehab therapists, behavioral health specialists, dietitians, recreation therapists, chaplains, and others to deliver wraparound care that engages older adults as they age in place, on their own terms.
