Job summary

Mercy is seeking an experienced Registered Nurse to join our team as a Nurse Navigator. In this role, you'll guide patients through their healthcare journey, coordinate care, and ensure a seamless experience from pre-treatment through follow-up care.

You will partner with patients, families, physicians, and nurses to assess needs, create care plans, educate patients, and support discharge planning in a hybrid/remote-friendly work environment.

Qualifications

• RN licensure in the state of practice is required.
• 2–3 years nursing experience minimum.
• Current BLS certification is required.
• ACLS and BSN are preferred qualifications.

Responsibilities

• Coordinate care for inpatient and outpatient populations.
• Guide patients through treatment journey; act as resource and advocate.
• Provide patient and family education including pre-op and follow-up care.
• Collaborate with physicians and multidisciplinary teams to ensure quality care.
• Support discharge planning and post-acute services coordination.
• Monitor progress to improve outcomes and patient satisfaction.
• Assist with LOS management and utilization review activities.
• Participate in quality improvement and best-practice initiatives.
• Serve as clinical resource and mentor to care team; document in EPIC.
