Job summary

MultiCare Health System in Tacoma, WA seeks a Nurse Navigator RN to coordinate care across the care continuum. The role blends nursing, case management and patient liaison duties to optimize outcomes, efficiency and patient experience. You will guide care plans, collaborate with nursing teams and support families through complex transitions.

Required are a WA RN license and 3–5 years clinical experience; an MSN/PhD/DNP/ARNP is preferred. This is a Day shift, with a flexible schedule.

Qualifications

• Bachelor's degree in Nursing is required or RN with 15+ years experience in lieu of education (depending on hiring date)
• Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
• Current WA Nursing license or MLS endorsement
• Certification in Oncology Nursing, Case Management, or related areas is highly desirable
• 3–5 years of recent clinical experience in the specialty area is required
• Current BLS certification is required

Responsibilities

• Organize patient care by prioritizing, delegating, delivering, and evaluating on an ongoing basis
• Use critical thinking to make decisions regarding patient care with reference to standards, resources and data
• Deliver nursing care following policies, procedures and protocols
• Collaborate with the nursing team to improve quality of care, patient experience and cost reduction
• Foster a learning environment for patients, families, staff and other healthcare team members
• Advocate for ethical, holistic care for diverse patient populations
• Track key quality metrics using nationally recognized benchmarks (where applicable)
• Lead care team initiatives to improve processes and outcomes aligned with the quadruple aim
• Address non-clinical barriers by connecting patients with community services

Education
