Job summary

UW Medical Center - Northwest is seeking a Part-time Nurse Care Coordinator to support the Care Management team in Seattle. The role focuses on coordinating patient flow, ensuring timely discharge, and collaborating with the interdisciplinary team to align care plans.

The position requires a Bachelor’s in Nursing, 3 years of nursing experience, and WA RN licensure, with Master’s preferred. Strong communication and leadership skills are essential for improving discharge efficiency and patient

Qualifications

Bachelor's degree in Nursing; Master's preferred.
Three years of professional nursing experience.
Current Washington State RN licensure.
Ability to communicate effectively in English, both verbally and in writing.

Responsibilities

Identification and monitoring of high medical needs patients to ensure timely completion of consults, procedures and tests, ongoing team communication, and facilitates coordination between providers across the continuum of care.
Lead medical team and nursing huddles and work creatively on removing barriers to discharge due to medical complications and/or system or team obstacles.
Assist with ordering complex medical equipment and in ensuring safe transitions of care.
Develop and monitor outcome measures related to care coordination and staff and patient satisfaction with discharge planning and providing ongoing consultation and education to staff on how to achieve improved patient flow through the UWMC system.
