Job summary

MultiCare Health System is seeking a Community Health Worker to join an interdisciplinary care team focused on care coordination and linking patients with community resources. You will inform patients about resources and help them navigate appointments and services.

You will travel to community sites as needed, educate peers, and participate in health fairs while supporting quality improvement initiatives and patient advocacy.

Qualifications

• High School Diploma or equivalent is required.
• 2 years of health care related experience preferred.
• WA Driver's License with a safe driving record per MHS standards.
• Experience in utilization management, community resources, and/or financial and quality issues preferred.
• Community Health Worker experience preferred.

Responsibilities

• Performs cross continuum care activities coordinating care between clinics and sites as appropriate.
• Engages in outbound communication to patients to keep them engaged in care and set up appointments.
• Helps patients access community resources and schedule appointments.
• Establishes contact with community partners; travel to sites is required.
• Develops tools to educate peers on patient advocacy and community resources.
• Participates in community health fairs and events.
• Educates the care team on available community resources.
• Participates in quality improvement activities.
