Job summary

MultiCare Health System's Tacoma General Hospital in Tacoma, WA is hiring a Care Coordinator Associate to join an interdisciplinary team supporting hospital transitions and patient-centered care.

In this role you will assist with cross-continuum care coordination, utilize data platforms, and help with scheduling, insurance authorizations, and resource coordination to ensure smooth throughput and seamless patient care.

Qualifications

• Post-secondary coursework, preferably in a health related field
• Bachelor’s degree highly desirable
• Minimum of 2 years experience in a health care setting, preferably as a Health Unit Coordinator or in a clinical role
• Experience in utilization management, community resources, and/or financial and quality issues affecting patient care desirable

Responsibilities

• The Care Coordinator Associate will support cross-continuum transitions as part of an interdisciplinary care team.
• Assist in ensuring smooth throughput and patient-centered care during transitions.
• Utilize multiple data platforms and troubleshoot as needed; handle scheduling, insurance and authorizations.
