Job summary

EMBASSY HEALTHCARE is seeking an MDS Coordinator to lead the development and completion of resident assessments (MDS) and care plans. The role requires coordinating with nursing staff and other departments to ensure timely assessments and compliant care planning.

You will oversee PPS timelines, participate in committee activities, and support discharge planning while maintaining strong documentation and adherence to regulations.

Qualifications

• Must possess, as a minimum, an LPN/RN Nursing degree from an accredited college or university.
• RN preferred.
• Must possess a current, unrestricted, active state license as a Licensed Practical Nurse or a Registered Nurse.
• Must be able to read, write, speak, and understand English.

Responsibilities

• Conduct and coordinate the development and completion of the resident assessment (MDS).
• Develop, implement and maintain an ongoing process for the resident assessment/care plans.
• Coordinate PPS schedules and ensure compliance with Medicare guidelines.
• Oversee interdisciplinary team meetings and care plan development.
• Assist with discharge planning and related documentation.
