Job summary

The MDS Coordinator role supports resident care by completing the RAI process for each resident, ensuring compliance with state and federal regulations and our internal policies. You will manage transmission and verification of receipts with the State and maintain quality standards.

As the MDS/Care Plan Coordinator, you will conduct care plan meetings, formalize written care plans, ensure staff awareness, and contribute to QA activities, committees, and reporting to the Director of Nursing

Qualifications

• Must be licensed as an RN or LPN in Texas.
• Minimum of one year of supervisory experience in a hospital, skilled nursing facility, or related healthcare program.
• Working knowledge of Long-Term care operational standards (Federal Register, Conditions of Participation, and State regulations).
• Ability to read and write in English.

Responsibilities

• Complete the RAI process for each resident in the facility.
• Conduct care plan meetings and formalize written care plans.
• Ensure staff involved in resident care are aware of the care plan and that plans guide daily care.
• Present QA information to the Director of Nursing Administration at monthly meetings.
