Gardant-Management-Solution in Illinois seeks a nursing supervisor to lead clinical staff, perform occasional hands-on care, and coordinate resident care. You will supervise a clinical team, ensure infection control, and oversee medication management while maintaining high-quality resident experiences. Qualifications

• Current state nursing licensure is required (LPN preferred).
• Minimum of two years nursing supervisory experience.
• Senior/supportive living experience strongly preferred.

Responsabilités

• Develops individualized care plans based on resident needs and preferences.
• Delivers health promotion, disease prevention, and self-care education to residents.
• Approximately 20% of hours provide direct hands-on nursing care.
• Engages with residents, families, and physicians to coordinate care.
• Hires, trains, and leads the clinical team to ensure high-quality care.
• Oversees infection control and medication management programs.
• Contributes to QA and performance improvement initiatives.
• Maintains confidentiality and privacy of resident information.

Responsibilities:Conducts initial assessment of prospective residents, including off-site visitsPerforms wellness assessments and develops individualized care plans based on the needs and preferences of each residentDelivers health promotion services, offers disease prevention counseling, and provides self-care education to residentsApproximately 20% of hours will be providing hands-on nursing responsibility and direct careEngages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experienceFosters communication by regularly engaging with physicians, pertinent healthcare providers, and family membersHires, trains, schedules, and leads the clinical team in providing exceptional care experiencesCoordinates the Community’s infection control program while maintaining state regulatory complianceDevelop and oversee the Community’s medication management programContributes toward quality assurance and performance improvement initiativesComplies with all rules and regulations regarding confidentiality and privacy of resident informationQualifications:Maintains current state nursing licensure.
