Responsibilities

• Coordinate and manage care for complex patients with PCP and care team.

• Conduct comprehensive assessments and communicate patient needs and care plans.
• Track referrals and coordinate care across providers and settings.
• Engage patients and families in care planning and self-management.
• Connect patients with health center and community resources for access to care.
• Monitor, evaluate and document patient progress and risk status.
• Collaborate with multidisciplinary teams to ensure high-quality care.
• Review assigned patient lists and coordinate complex cases with care managers.
