Edward M. Kennedy Community Health Center in Worcester, MA seeks an RN Clinical Care Manager to coordinate evidence-based care for MAT patients and support the health needs of the community.

The CCM partners with Primary Care Providers to manage care plans, coordinate transitions, provide vaccinations and assessments, and document progress in the EHR. Qualifications include MA RN license, BSN, 1–2 years of clinical experience, CPR certification, and required COVID-19 booster and annual flu

Qualifications

• Registered Nurse licensed in Massachusetts.
• Bachelor’s degree in Nursing from an accredited program.
• 1-2 years of clinical nursing experience.
• Current CPR certification.
• COVID-19 booster and annual flu vaccine required.

Responsibilities

• Coordinate and manage care for MAT patients with PCPs to ensure optimal care and education.
• Manage care plans for transitions between facilities to prevent disease exacerbation and reduce hospital/ER visits.
• Identify and coordinate care, maintaining communication with the care team on patient status and needs.
• Provide direct patient care including assessments, vaccinations, medication reconciliation, and lab reviews.
• Involve patients and support systems in care planning; document progress in the EHR; evening/weekend/holiday shifts may be required.

Education

Registered Nurse License in MA

Bachelor’s degree in Nursing

Current CPR certification

1-2 years of clinical experience
