Franklin Wellness & Rehabilitation Center is seeking compassionate Nurses (LPN or RN) to join our care team in Franklin, Tennessee. You will direct daily activities of CNAs, participate in admissions and transfers, and ensure medication administration per physician orders.

This full-time role supports a team-driven environment. Requirements include an active Tennessee LPN or RN license and CPR/BLS certification.

Qualifications

• Current CPR/BLS certification.
• Reliability and compassionate patient care in a team setting.

Responsibilities

• Direct daily activities of CNAs and ensure policy adherence.
• Assist with admissions, discharges and transfers of residents.
• Assess residents, prepare and administer medications per orders.
• Initiate, review and update care plans for residents.
• Report changes to the nurse supervisor and document factors affecting work.
• Maintain confidentiality of protected health information.

Nurse Position Summary

As a Nurse (LPN or RN) at Franklin Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

Nurse Requirements

• Current unrestricted LPN or RN License for the state of Tennessee
• Current CPR/BLS Certification
