Job summary

Phoenix House of New York in Long Island City is seeking a Licensed Practical Nurse to provide direct nursing care within a treatment setting focused on recovery. You will support clients as they work toward stability, dignity, and lasting change.

You will administer prescribed medications, monitor vital signs, document assessments, coordinate medical appointments, and collaborate with the treatment team to ensure timely follow-up and high-quality care while maintaining patient confidentiality

Qualifications

• High School Diploma or GED required.
• Graduate of an accredited nursing program.
• Current NYS LPN license.
• First Aid, CPR, Narcan, Infection Control and Child Abuse certifications required.

Responsibilities

• Provide direct nursing care to clients under supervision of clinical leadership.
• Administer prescribed medications (oral, topical, injectable) per NYS scope of practice.
• Document vital signs, assessments, and patient condition.
• Monitor patient status and report significant changes to leadership.
