Job summary

Phoenix House invites a Licensed Practical Nurse to join our Long Island City team. You will provide direct nursing care, monitor patients, administer medications, and collaborate with the treatment team to ensure consistent care and follow-up.

The role emphasizes compassion, attention to detail, and support for recovery in a structured environment. NYS LPN license and experience with chemically addicted patients are valuable assets.

Qualifications

• Licensed Practical Nurse (LPN) with NYS license.
• Experience caring for chemically dependent patients and withdrawal scenarios is beneficial.
• Strong patient-focused care and documentation skills.

Responsibilities

• Provide direct nursing care to clients under supervision.
• Administer prescribed medications as per policy (oral, topical, injectable).
• Monitor vital signs and document clinical assessments.
• Coordinate medical appointments and respond to health concerns.
• Oversee unlicensed assistive personnel (CNAs, MAs, Techs).
• Maintain patient confidentiality and professional boundaries.
• Educate patients on treatment plans and self-care.

The Licensed Practical Nurse Will Perform The Following Tasks

• Provide direct nursing care to clients under the supervision and direction of clinical leadership.
• Maintain patient confidentiality in accordance with Phoenix House policies and procedures.
• Oversee unlicensed assistive personnel (e.g., CNAs, Medical Assistants, and Medical Technicians).
• Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies.
• Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership.
• Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership.
• Engage in health education and reinforce treatment plans, medication adherence, and self‑care strategies.
• Other duties as assigned.

REQUIRED EDUCATION

• High School Diploma or GED
• Graduate of an accredited nursing program

LICENSES/CERTIFICATIONS

• Current NYS Licensed Practical Nurse

Required Experience

• At least three years of related experience
• Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugs.
