Responsibilities

• Provide direct nursing care to clients under supervision of clinical leadership.
• Maintain patient confidentiality per policies.
• Oversee unlicensed assistive personnel (CNAs, medical assistants, technicians).
• Perform routine nursing procedures (EKGs, wounds, specimen collection) and administer medications.
• Document vital signs and report significant findings to leadership.
• Observe and report changes in patient health status promptly.
• Engage in health education and reinforce treatment plans and self-care.

Job Description

Phoenix House is seeking a Licensed Practical Nurse to support clients’ health, safety, and recovery in a behavioral health and substance use treatment setting. In this role, you will help ensure clients receive attentive medical support and compassionate care throughout their treatment experience.

As part of a dedicated care team, you will monitor client health needs, support medication processes, document care, coordinate medical follow-up, and help create a safe, structured environment where clients can focus on healing and recovery.

The Licensed Practical Nurse will perform the following tasks:

• Provide direct nursing care to clients under the supervision and direction of clinical leadership.
• Maintain patient confidentiality in accordance with Phoenix House policies and procedures.
• Oversee unlicensed assistive personnel (e.g., CNAs, Medical assistants, and Medical Technicians).
• Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal)in accordance with NYS scope of practice and facility policies.
• Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership.
• Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership.
• Engage in health education and reinforce treatment plans, medication adherence, and self-care strategies.
• Other duties as assigned.

Qualifications

REQUIRED EDUCATION

• High School Diploma or GED
• Graduate of an accredited nursing program

REQUIRED EXPERIENCE

• At least three years of related experience
• Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugs.

• Proficiency in transcribing medication orders to the pharmacy and or the EMR
• Proficiency of data collection of newly admitted clients

WORK ENVIRONMENT

This job operates in an indoor medical clinical environment. This role also routinely comes into contact with patients who may have contagious illnesses.
