Job summary

Phoenix House of New York is seeking a Licensed Practical Nurse for our Long Island City residential program. You will provide direct nursing care, assist with medications, monitor vital signs, and support clients on their recovery journey in a calm, compassionate, and professional manner.

Qualifications

• High School Diploma or GED required and graduate of an accredited nursing program.
• Current NYS Licensed Practical Nurse, First Aid, CPR, Narcan, Infection Control Certification, and Child Abuse Certification.
• At least three years of related nursing experience, preferably with chemically addicted patients or withdrawal management.

Responsibilities

• Provide direct nursing care under clinical leadership.
• Monitor vital signs and document assessments.
• Administer prescribed medications (oral, topical, injectable) per NYS scope of practice.
• Oversee unlicensed staff (CNAs, Medical Assistants, Technicians).
• Coordinate medical appointments and health screenings; ensure follow-up care.

The Licensed Practical Nurse will perform the following tasks:

• Provide direct nursing care to clients under the supervision and direction of clinical leadership.
• Maintain patient confidentiality in accordance with Phoenix House policies and procedures.
• Oversee unlicensed assistive personnel (e.g., CNAs, Medical Assistants, and Medical Technicians).
• Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies.
• Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership.
• Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership.
• Engage in health education and reinforce treatment plans, medication adherence, and self-care strategies.
• Other duties as assigned.

REQUIRED EDUCATION
