Job summary

Phoenix House, a nonprofit provider on Long Island, seeks a Licensed Practical Nurse to support clients through recovery with direct patient care, medication management, and health screenings. You will document vital signs, monitor progress, and coordinate care under clinical leadership.

Qualifications include NYS LPN licensure, 3+ years of experience with chemically addicted patients, and relevant certifications.

Qualifications

• Current NYS Licensed Practical Nurse (LPN)
• First Aid, CPR, Narcan, Infection Control Certification, and Child Abuse Certification
• At least three years of related experience, including care for chemically addicted patients or patients withdrawing from substances

Responsibilities

• Provide direct nursing care to clients under supervision and direction of clinical leadership.
• Maintain patient confidentiality per Phoenix House policies.
• Oversee unlicensed assistive personnel (CNAs, Medical Assistants, Technicians).
• Perform routine nursing procedures (EKGs, wound care, specimen collection).
• Administer prescribed medications (oral, topical, injectable) per NYS scope of practice and facility policies.
• Document vital signs and patient assessments; report significant findings to leadership.
• Observe and report changes in patient condition promptly to clinical leadership.
• Engage in health education, reinforce treatment plans, medication adherence, and self-care strategies.
• Other duties as assigned.

The Licensed Practical Nurse Will Perform The Following Tasks

• Provide direct nursing care to clients under the supervision and direction of clinical leadership.
• Maintain patient confidentiality in accordance with Phoenix House policies and procedures.
• Oversee unlicensed assistive personnel (e.g., CNAs, Medical Assistants, and Medical Technicians).
• Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies.
• Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership.
• Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership.
• Engage in health education and reinforce treatment plans, medication adherence, and self-care strategies.
• Other duties as assigned.

Qualifications

REQUIRED EDUCATION

• High School Diploma or GED
• Graduate of an accredited nursing program
