Qualifications

• Certified in BLS/ACLS.

Responsibilities

• Coordinate and facilitate safe patient discharges.
• Arrange follow-up appointments and post-discharge outreach.
• Coordinate prescriptions and transportation prior to discharge.
• Provide discharge education including meds and weight monitoring.
• Collaborate with care teams to support transitions of care.
• Identify barriers delaying discharge for Heart Failure patients.
• Document activities in medical record and track quality metrics.
• Support Heart Failure management initiatives to reduce readmissions.
