Job summary

Grady Health System is seeking a Discharge Nurse Coordinator to lead coordinated discharge planning in collaboration with the nursing and medical teams. You will ensure timely discharge, arrange post-discharge services, and educate patients and families on follow-up care, medications, and home needs.

The role emphasizes seamless communication with patients, families, and care teams, participation in interdisciplinary rounds, and ongoing improvement of discharge processes and quality outcomes.

Qualifications

Registered Professional Nurse with active licensure (RN).
Clinical experience with NICU, at least two years.
Basic Life Support certification (AHA).

Responsibilities

Coordinate discharge planning with interdisciplinary team to ensure timely discharge.
Arrange post-discharge services and follow-up care.
Ensure follow-up appointments are scheduled prior to discharge.
Communicate with patients and families to develop mutually agreed care plans.
Participate in daily rounds and discharge planning meetings.
Monitor discharge process and follow-up calls.
Coordinate with Home Health, Hospice, and other agencies for needed services.
