Saint Francis Health System invites applications for a Nurse Navigator in Heart Failure. The role supports time-sensitive cardiac nursing expertise and clinical oversight for heart failure patients within the hospital, coordinating care from admission through discharge.

Key responsibilities include reviewing multidisciplinary care per current guidelines, educating patients on plan of care, and coordinating post-discharge follow-up while working with physicians and the care team.

Formación

• Has completed basic professional nursing curricula and holds/entitled to hold a diploma or degree.
• Valid multi-state or State of Oklahoma RN license. Heart Failure Society of America Optimal Medical Therapy certification within 6 months of hire.
• Minimum three years of clinical nursing experience, including at least one year in critical care (preferred).
• Knowledge of guideline-directed medical therapy, devices, remote monitoring, and post-discharge care management.

Responsabilidades

• Concurrently review the care of emergent and acute heart failure patients per latest guidelines.
• Perform socio-economic evaluation related to medications and payor sources to optimize adherence.
• Establish patient relationships and assess understanding of the plan of care and medications.
• Maintain accurate documentation and coordinate referrals with care team.
• Oversee continuity of care and serve as a point of contact during hospitalization.
• Coordinate post-discharge heart failure follow-up activities and education.
