Job summary

Grady Health System is seeking a Discharge Nurse Coordinator in Atlanta to collaborate with nursing, medical staff, and the interdisciplinary team to assure effective, timely discharge following continuation of care. The role emphasizes coordinating post-discharge needs, education, and follow-up planning to optimize patient outcomes.

Qualifications

• Active RN license in Georgia.
• BLS certification (American Heart Association).
• Bachelor’s degree in nursing.
• Minimum 2 years NICU clinical experience.

Responsibilities

• Coordinate discharge planning with the interdisciplinary team.
• Ensure timely discharge and arrange follow-up care and appointments.
• Communicate with patients/families to develop and implement care plans.
• Participate in daily interdisciplinary rounds and discharge planning meetings.
• Coordinate with social workers, case management, and referral agencies for post-discharge needs.
• Develop and evaluate education and quality improvement projects related to discharge.
• Monitor discharge process metrics and report findings to unit leadership.
