Job summary

Grady Health System in Atlanta seeks a Nursing Discharge Navigator to coordinate safe, efficient discharges, arrange follow-up care, reconcile medications, and ensure transportation. This role emphasizes early discharge planning for Heart Failure patients, collaborating with physicians, case management, pharmacy, social work and therapy to reduce length of stay and readmissions.

Candidates should have RN licensure in Georgia, BSN preferred, and ACLS/BLS credentials, with 3 years of clinical

Qualifications

• RN with Nurse Case Management experience and 3+ years of clinical nursing experience.
• BSN preferred; Registered Nurse in Georgia.
• American Heart Association BLS and ACLS certifications required.

Responsibilities

• Coordinate and schedule follow-up appointments prior to discharge.
• Conduct discharge calls to assess understanding and address concerns.
• Reconcile and coordinate prescriptions and ensure medication access.
• Provide comprehensive discharge education on medications, monitoring, diet, and when to seek care.
• Confirm transportation arrangements for safe discharge home.
• Collaborate with physicians, case management, pharmacy, social work, and therapy services to support timely transitions of care.
• Identify barriers to discharge early and streamline processes to reduce length of stay for Heart Failure patients.
• Document activities in the medical record and track quality metrics.
