Grady Health System in Atlanta, GA is seeking a Nursing Discharge Navigator to coordinate safe, timely discharges, arrange follow-ups, prescriptions, and transportation, with a focus on Heart Failure patients to reduce length of stay and readmissions.

You will collaborate with physicians, case management, pharmacy, social work, and therapy services to ensure smooth transitions of care, provide discharge education, and track quality metrics.

Competenze

• Nurse Case Management experience.
• 3 years of clinical nursing experience.
• Bachelor's degree in Nursing.
• Registered Professional Nurse licensed in Georgia.
• BLS and ACLS certifications from the American Heart Association.

Mansioni

• Coordinate and schedule follow-up appointments with primary care providers and specialists prior to patient discharge.
• Conduct discharge phone calls to assess patient understanding and ensure post-discharge needs are met.
• Ensure prescriptions are reconciled, called in, and accessible to patients prior to discharge.
• Provide clear discharge education including medication instructions, symptom monitoring, diet, weight monitoring, and when to seek care.
• Confirm transportation arrangements are secured prior to discharge for safe return home.
• Collaborate with physicians, case management, pharmacy, social work, therapy services and other departments for safe transitions of care.
• Identify and address potential barriers early in hospitalization that may delay discharge.
• Work with the interdisciplinary team to shorten length of stay for Heart Failure patients through early education and planning.
• Support Heart Failure management initiatives to reduce readmissions.
• Document discharge coordination activities in medical records.
• Track quality metrics related to length of stay, readmissions, and follow-up compliance.
