Atrium Health Specialty Network, Inc. is seeking a registered nurse to lead discharge planning, coordinating with physicians to develop patient-centered plans and address barriers to timely transitions.

The role requires BSN preferred, 3 years nursing experience (ICU/ED preferred), and current BLS; you will educate patients and families, arrange follow‑ups, and support performance improvement initiatives.

Qualifications

• Graduate from an accredited School of Nursing.
• 3 years nursing experience; ICU or ED experience preferred.
• Current Basic Life Support (BLS) for Healthcare Providers.
• RN license in the state of work or eligibility via compact licensure.
• BSN preferred.

Responsibilities

• Create focused discharge plans addressing barriers for long LOS.
• Coordinate evaluation, testing, management and follow‑up with the attending physician and team.
• Rounds with physicians and liaises between physician, team, patient and family.
• Promote patient and family education to support continuity of care at discharge.
• Ensure patients have scheduled follow‑up appointments prior to discharge.
• Participate in performance improvement; analyze and interpret data.
• Assist with prioritizing and assigning patients to correct providers.
• Coordinate care outside the hospital for non‑hospitalized patients to prevent readmissions.
• Serve as a resource to clinical staff for discharge-related issues.
• AnswerCalls: pharmacy, patient, family post discharge to bridge transitions.
• Follow up on pending labs/procedures/tests after discharge.
