Job summary

Atrium Health is seeking an experienced Registered Nurse Case Manager to identify patients needing discharge planning, assess needs, and coordinate with the care team to achieve optimal outcomes. You will lead care planning, collaborate with physicians and social services, and document progress in the EMR.

The role requires BSN, 5+ years clinical experience, and RN licensure in the state of practice, with MSN preferred and case management certification encouraged within 2 years.

Qualifications

• BSN required.
• MSN preferred.
• Minimum 5 years of relevant clinical experience.
• Case management experience preferred.

Responsibilities

• Identify patients who would benefit from Case Management interventions.
• Assess data by interviewing patients and family and evaluating needs.
• Develop and implement Plan of Care with nursing to achieve optimal outcomes.
• Monitor discharge planning effectiveness and adjust Plan of Care as needed.
• Document actions in the EMR and communicate with providers/payors.
• Participate in rounds and coordinate with physicians and community services.
• Identify system issues and propose interventions to reduce duplication of services.
