Responsibilities

• Conducts comprehensive assessments and develops care plans.
• Coordinates discharge planning and transfers with patient and family.
• Collaborates with social work, physicians, and care teams to resolve barriers.
• Documents UM activities and communicates with payers and external reviewers.
• Acts as educator and resource on admission status and regulatory requirements.
