Job summary

Advocate Health, a large nonprofit health system, is seeking a full-time RN within the Union Hospitalists group in North Carolina to support discharge planning and care coordination with physicians and the healthcare team in Union, North Carolina.

You will create focused discharge plans, ensure follow-up appointments, educate patients and families, participate in performance improvement, and help coordinate post-discharge care to prevent readmissions.

Qualifications

• Graduate from an accredited School of Nursing.
• Current Basic Life Support (BLS) for Healthcare Provider status according to American Heart Association.
• RN license in the state where you work or eligibility for National license compacts.

Responsibilities

• Creates focused patient-centric discharge plans addressing barriers to discharge.
• Coordinates evaluation, testing, management and follow-up with attending physician and care team.
• Rounds with physicians; liaises between physician, healthcare team, patient and family.
• Promotes patient and family education to support treatment plan and continuity of care.
• Ensures follow-up appointments are scheduled prior to discharge.
• Participates in performance improvement; analyzes data.
• Prioritizes and assigns patients to providers.
• Coordinates post-discharge care for non-hospitalized patients to prevent readmissions.
• Serves as a resource to clinical staff and answers post-discharge inquiries.
