Job summary

Advocate Health is seeking a Registered Nurse to join the Union Hospitalist discharge team. You will collaborate with providers to craft focused discharge plans, coordinate tests and follow-up, and serve as a liaison among patients, families and the care team.

The role requires a BSN or progress toward it, at least three years of nursing experience (ICU/ED preferred), and current RN licensure with BLS. 40 hours/week, daytime shifts, with a pay range of $38.20–$57.30 per hour.

Qualifications

• Graduate from an accredited School of Nursing.
• BSN preferred.
• 3 years nursing experience.
• ICU or ED experience preferred.
• Current RN license in the work state; BLS required.

Responsibilities

• Collaborate with providers to create discharge plans addressing barriers to discharge.
• Coordinate evaluation, testing, management and follow-up with the care team.
• Rounds with physicians and liaison between physician, patient, family, and care team.
• Educate patients and families to support treatment plans and transitions to home or next care setting.
• Ensure follow-up appointments are scheduled prior to discharge.
• Participate in performance improvement activities and data analysis.
• Prioritize and assign patients to appropriate providers.
• Coordinate care post-discharge for non-hospitalized patients to prevent readmissions.
• Provide post-discharge support via phone for labs/procedures/tests.
