Qualifications

• Active Illinois RN license required; 3–5 years clinical experience.
• Residence in or travel within Rock Island, Mercer, Whiteside counties area.
• Willingness to travel 50–75% for in-person case management.
• Strong analytical, problem‑solving, and communication skills.
• Proficiency with standard office software (Word, Excel, Outlook, PowerPoint).
• Ability to work independently and collaborate across teams.

Responsibilities

• Coordinate case management for catastrophic and chronically ill members.
• Assess needs, plan care, facilitate services, and advocate for options.
• Communicate with members, providers, employers, and insurers.
• Document case work and monitor progress toward outcomes.
• Interact with multidisciplinary teams to optimize wellness and return to baseline.
• Travel to member homes, worksites, or physician offices as needed.
