Your Health is seeking a Registered Nurse to serve patients throughout the Andrews area as a Nurse Case Manager. The role visits patients, facilitates with providers, and initiates telemedicine while ensuring quality care in compliance with laws and scope of practice.

The position is full-time, salary-based with 8-hour shifts from Monday to Friday (8am–5pm). Travel between offices and facilities is required, and a valid driver's license is essential.

Qualifications

• Must be a licensed nurse with active license.
• RN license preferred; license must be in good standing.
• Minimum of three (3) years clinical experience.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with care teams, patients, and caregivers.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to demonstrate leadership and work well in teams.
• Willingness to travel between offices, facilities, etc.; valid driver’s license and insured vehicle.

Responsabilités

• Visit patients in the Andrews area and coordinate with providers.
• Evaluate patients and initiate telemedicine visits.
• Review medical records and complete consents.
• Enroll patients in Care Management if eligible.
• Develop and monitor a Care Plan of Care for eligible patients.
• Document Care Management activities and coordinate with care teams.
• Coordinate with patient’s health care team, facilities, and families.
• Maintain compliant practice under laws and organizational policies.
• On-call may be required.
