Job summary

Your Health is seeking a Clinical Coordinator to support the Andrews clinic. The role focuses on chart prep, scheduling visits, and logging care management activities to ensure smooth patient care in a fast-paced value-based setting.

The position is full-time, 8-hour shifts from 8:00 AM to 5:00 PM, Monday through Friday, with travel between offices as needed. A CMA or licensed nurse with CPR and strong communication skills is required.

Qualifications

• Must be CMA or licensed nurse with active license in SC.
• Minimum 1 year experience in a healthcare setting.
• CPR certification required.
• High school diploma or equivalent.
• Strong interpersonal and communication skills.
• Ability to work flexible hours and travel between offices.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all necessary documentation and test results are accessible for the provider.
• Maintain accurate up-to-date patient records with demographics and contact details.
• Ensure confidentiality and privacy regulations adherence; verify consent forms.
• Reconcile medications.
• Coordinate patient appointments with providers and care team.
• Interact with patients via email, portal, phone, text, or in person to schedule visits and provide pre-appointment instructions.
• Document and log care management activities.
