Job summary

Your Health Organization is seeking a Registered Nurse to serve patients throughout designated service areas. The Nurse Case Manager will visit homes and facilities, coordinate with providers, and initiate telemedicine visits as needed.

Reliable transportation and daily travel are required, with a full-time, 8-hour shift schedule (8am-5pm). The role involves managing care plans, evaluating patients for home health or RPM options, and collaborating with the care team to ensure quality outcomes.

Qualifications

• Must be a licensed nurse in good standing with the nursing board.
• Registered nurse preferred; licensure must be current.
• Experience collaborating with multidisciplinary care teams is expected.

Responsibilities

• Facilitate receiving medical records from patient providers and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management if eligible and initiate Plans of Care.
• Coordinate care and communicate with providers, patients, and families.
• Reconcile medications and provide post-discharge education.
• Travel to patients’ homes and facilities as required.
