Job summary

Your Health Organization in the Andrews area is hiring a Registered Nurse as a Nurse Case Manager to support patients in clinics and homes. The role includes home visits, telemedicine coordination, and ensuring high-quality care across primary, specialty, and care management services.

The Nurse Case Manager will collaborate with interdisciplinary teams, review records, enroll patients in Care Management, and facilitate plan-of-care development while maintaining compliance with laws and policies.

Qualifications

• Must be a licensed nurse with an active RN license.
• Minimum three years of clinical experience preferred.
• Experience in community settings preferred.
• Ability to communicate and collaborate with care teams, patients, and caregivers.
• Strong written and verbal communication skills.
• Basic computer knowledge.
• Ability to lead and manage multiple responsibilities with sound judgment.
• Must be able to travel between offices and facilities; reliable transportation required.
• Liberal driving requirements: licensed driver with insured vehicle.

Responsibilities

• Facilitate receiving all medical records from the patient’s primary provider and specialists.
• Review medical records and complete consents with patients.
• Enroll patients in Care Management if eligible and initiate a Care Management Plan of Care.
• Coordinate with the patient’s care team and ensure care plans are up to date.
• Evaluate for home health, hospice, or RPM resources as needed.
