Job summary

Your Health Organization is recruiting a Registered Nurse to serve patients in the Conway area. The Nurse Case Manager will visit patients, initiate telemedicine visits with the clinic, and support care team coordination according to applicable laws and policies.

The role requires daily travel and offers a full-time, salary-based schedule (8-hour shifts, Monday–Friday, 8am–5pm). We serve facilities, homes, clinics, and virtual visits with comprehensive primary care and specialty services.

Qualifications

Must be a licensed nurse with good standing.
Minimum of three (3) years clinical experience preferred.
Experience in community settings preferred.
Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.

Responsibilities

Facilitate receiving and review of medical records from the patient’s primary provider and specialists.
Enroll patients in Care Management, if they meet eligibility criteria.
Initiate a Care Management Plan of Care, if the patient is eligible.
Coordinate with the patient’s health care team and other care providers.
Document Care Management activities accurately and keep plans up to date.
Assist in identifying resources such as home health, RPM devices, or other tools to improve patient outcomes.
