Job summary

Your Health is seeking a Registered Nurse to join our growing Care Management team serving the Service Area. The Nurse Case Manager will visit patients in homes and facilities, coordinate with providers, and initiate telemedicine visits, traveling daily as needed.

This full-time, salary-based role requires a valid RN license, at least three years of clinical experience, and strong communication skills to work with interdisciplinary teams.

Qualifications

• Must hold a current RN license in good standing.
• Minimum three (3) years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with interdisciplinary care teams.

Responsibilities

• Facilitate receiving all medical records from patient’s providers.
• Review medical records and complete consents.
• Enroll patients in Care Management if eligible.
• Initiate Care Management Plan of Care for eligible patients.
• Coordinate with care teams and document Care Management activities.
• Evaluate home health, hospice, palliative needs, or RPM resources.
• Travel to patients' homes and facilities as required.
