Job summary

Your Health in the Augusta area is seeking a Registered Nurse as a Visiting Nurse (Nurse Case Manager) to support patients in homes and facilities. You will travel daily to patients’ residences and long-term care settings, initiating telemedicine visits and coordinating care with providers.

The role is full-time, with 8-hour shifts from Monday to Friday (8am–5pm). Responsibilities include reviewing records, enrolling patients in Care Management, and developing Plans of Care.

Qualifications

• Must be a licensed nurse (RN preferred) with a valid nursing license.
• Minimum of three (3) years clinical experience.
• Experience in community/long-term care settings preferred.
• Proven ability to communicate effectively with patients, families, and care teams.
• Strong written and verbal communication skills; proficient with basic computer use.

Responsibilities

• Facilitate receiving medical records from patient providers and specialists.
• Review medical records and ensure completeness.
• Complete consents and enroll patients in Care Management if eligible.
• Initiate and monitor Care Management Plan of Care for eligible patients.
• Capture diagnoses accurately and address gaps in documentation.
• Complete AWV’s and cognitive impairment screenings.
• Perform Social Determinants of Health assessments and screenings.
• Document Care Management activities and maintain up-to-date plans.
• Coordinate care across home health, hospice, and RPM resources.
• Travel to patients' homes and facilities; participate in on-call rotation.
