Your Health is seeking a Registered Nurse to join our Visiting Nurse team serving the 29229, 29206, 29205 area as a Nurse Case Manager. You will visit patients in homes and facilities to evaluate needs and facilitate with providers, traveling daily as required.

As a critical care team member, you will coordinate care plans, enroll patients in management programs, review records, and support telemedicine visits while complying with laws and policies. Full-time, 8-hour shifts, Monday–Friday.

Qualifications

• Must be a licensed nurse with a good standing license.
• Minimum of three years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with care teams, patients, and caregivers.
• Strong written and verbal communication skills.
• Basic computer knowledge.
• Ability to lead, coordinate care, and work with multiple duties.
• Ability to travel between offices, facilities, and patient homes with reliable transportation.

Responsabilités

• Receive and review medical records from primary providers and specialists.
• Enroll patients in Care Management if eligible and initiate a Plan of Care.
• Capture diagnoses with specificity and address gaps.
• Complete AWV’s and cognitive impairment screenings as needed.
• Perform SDoH assessments and ACPs with patients and providers.
• Evaluate for home health, hospice, or RPM needs and coordinate with divisions.
• Coordinate care and reconcile medications and therapies.
• Provide post-discharge education and ensure care plans are up to date.
• Assess for adaptive equipment, DME, and safe home environments.
• Document Care Management activities accurately; on-call may be required.
