Job summary

Manchester Memorial Hospital, Inc. is seeking a Clinical Nurse Educator to oversee care management and discharge planning across the Greater Manchester region.

The role coordinates patient transitions, collaborates with the interdisciplinary team, and ensures age-appropriate care across the life span. A strong background in medical–surgical nursing, case management, and home care supports success in this position.

Qualifications

• Minimum five (5) years of nursing experience in a medical–surgical acute care setting.
• Additional experience in case management, home care, and utilization review/management is preferred.
• Current Connecticut RN license and active practice, with professional certification preferred.

Responsibilities

• Links providers, payers, physicians, and the community to coordinate patient care transitions.
• Monitors admissions for appropriateness of observation vs. inpatient care per InterQual criteria.
• Coordinates discharge planning and home or nursing facility services using nursing judgment.
