You will coordinate discharge planning, mentor nursing staff in evidence-based practices, and collaborate with physicians, case managers and home health teams to ensure smooth transitions of care. The role requires a CT RN license, a BSN, at least five years of medical-surgical nursing and preferred case management

Qualifikationen

• Graduate of an accredited school of Nursing.
• Baccalaureate degree in nursing required.
• Current CT RN license.
• Case management or clinical specialty certification preferred.

• Coordinate discharge planning and ensure appropriate care transitions.
• Provide link between provider and payer organizations and the community.
• Mentor nursing staff in evidence-based practices and care management.
• Communicate patient care needs to interdisciplinary team.
