Job summary

Hartford HealthCare’s Greater Manchester Region is seeking a Clinical Nurse Educator to lead patient care coordination, discharge planning, and professional nursing practice across facilities.

The role requires CT RN licensure, a BSN, and at least five years of medical–surgical nursing experience. Certification in case management or clinical specialty is preferred. Willingness to cover at Manchester Memorial Hospital as needed and report to the Care Management Manager.

Qualifications

• Must be a Graduate of an accredited school of Nursing.
• BSN degree required; CT RN license active.
• Professional certification in case management or clinical specialty preferred.

Responsibilities

• Monitor admissions for appropriateness of observation or inpatient care.
• Coordinate discharge planning and home or facility services.
• Provide nursing judgment and interdisciplinary collaboration.
• Communicate patient condition and needs to care team.
• Promote nursing practice and professional development.
• Provide coverage at Manchester Memorial Hospital as needed; report to Care Management Manager.
