Job summary

The Manchester Memorial Hospital, Inc. is seeking a Care Manager to bridge provider and payer organizations and coordinate patient care transitions.

You will monitor admissions, coordinate discharge planning and collaborate with the interdisciplinary team to ensure appropriate care settings. This role requires a nursing background with CT licensure and a BSN preferred, plus at least five years of med-surg experience and strong analytical and communication skills.

Qualifications

• RN with CT license required.
• BSN degree preferred.
• Graduate of accredited nursing program.
• 5+ years of nursing experience in med-surg.
• Case management/home care/utilization review experience preferred.

Responsibilities

• Coordinate discharge planning and post-acute services.
• Monitor admissions for observation vs. inpatient care.
• Collaborate with physicians, payers and the care team.
• Communicate patient needs and clinical conditions.
• Provide coverage at both hospitals as needed.
