The role requires a Connecticut RN license, a BSN (or applicable field), and 5+ years of medical-surgical experience. Certification in case management or related specialty is preferred. The nurse will collaborate with the care team and may work across both campuses as needed.

Qualifikationen

• Must be a graduate of an accredited school of Nursing.
• Current license as a Registered Nurse in the State of Connecticut.
• Bachelor's degree in an applicable field is required.
• Professional certification in case management or clinical specialty is preferred.

Aufgaben

• Coordinate patient care across the care continuum, including transitions to home or long-term care settings.
• Monitor admissions for appropriateness of observation vs. inpatient care using InterQual criteria.
• Communicate patient condition and care needs to the interdisciplinary team and families.
• Utilize nursing judgment to coordinate services and ensure age-appropriate care for the patient population.
• Provide coverage at both hospitals as needed and report to Care Management leadership.
