Job summary

Hartford HealthCare is seeking a Clinical Nurse Educator/ Care Manager for the Greater Manchester Region at Manchester Memorial Hospital. You will link providers, payers and patients, coordinating discharge planning and community services.

The role requires nursing leadership, five+ years in medical-surgical care, and CT RN licensure with possible CM certification. Expect collaboration with an interdisciplinary team and coverage across facilities.

Qualifications

Graduate of an accredited school of Nursing.
BSN or higher preferred; RN licensure in Connecticut required.
Professional certification in case management or a clinical specialty is preferred.
Five or more years of nursing experience in medical-surgical acute care; case management/home care/utilization review experience preferred.
Strong communication, analytical and interpersonal skills; proficient English; basic PC skills.

Responsibilities

Coordinate patient care transitions between providers, payers and the community.
Monitor admissions for appropriateness of observation and inpatient care per InterQual criteria.
Identify discharge planning needs and arrange services at home or in a nursing facility.
Collaborate with the interdisciplinary team and report to Care Management leadership.
