The role uses InterQual criteria to assess admission needs and guides discharge planning for home or facility settings. You will apply nursing judgment, communicate patient needs to the interdisciplinary team, and provide age-appropriate care.

Qualifications

• Graduate of an accredited school of Nursing.
• BSN degree required.
• Current RN license in the State of Connecticut.
• Professional certification in case management or clinical specialty preferred.

Responsibilities

• Coordinate patient care across providers, payers, and the community for transitions of care.
• Monitor admissions for appropriateness per InterQual criteria.
• Identify discharge planning needs and coordinate home or nursing facility services.
• Communicate patient’s clinical condition and care needs to the interdisciplinary team.
• Apply nursing judgment and promote age-appropriate care across the patient population.
• Provide coverage at both Manchester Memorial Hospital and Rockville General as needed.
