Job summary

Manchester Memorial Hospital, Inc. is seeking a Clinical Nurse Educator to coordinate patient care transitions and collaborate with providers, payers, and the community to ensure appropriate admission decisions and discharge planning. The role requires strong nursing judgment and interdisciplinary collaboration.

The Educator will support growth within a Hartford HealthCare network and ensure age-appropriate care. A CT RN license and BSN are required, with case management certification preferred.

Qualifications

• Graduate of an accredited school of Nursing.
• Baccalaureate degree in an applicable field required.
• Current CT RN license required.
• Professional certification in case management or clinical specialty preferred.

Responsibilities

• Coordinate patient care transitions with providers, payers, and community resources.
• Monitor admissions for appropriateness per InterQual criteria.
• Use nursing process to identify discharge planning needs and coordinate home or facility care.
• Communicate patient condition and care needs to the interdisciplinary team.
• Provide age-appropriate care and professional nursing practices across life span.
