Job summary

Living Branches is seeking a full-time PC Care Coordinator to coordinate resident care services, assess needs, and maintain care plans. You will guide staff, manage documentation, and liaise with physicians and families to ensure high-quality care.

Required RN or LPN license and CPR certification, with 2–4 years of nursing experience. Knowledge of HIPAA and long-term care regulations is essential; strong communication and leadership skills are a must.

Qualifications

• RN or LPN license required.
• CPR certification required.
• 2–4 years nursing experience preferred.
• Knowledge of HIPAA rules and long-term care regulations.
• Strong managerial and communication skills for guiding the team.

Responsibilities

• Coordinate resident care services, assessments, care planning and re-evaluations to ensure quality care.
• Provide direction to assigned staff and monitor performance.
• Schedule physician visits and arrange transportation; send medical records as needed.
• Participate in QA processes and ensure regulatory documentation compliance.
