Job summary

Ascension Medical Group Unite Here Health in downtown Chicago seeks a Board Certified Nurse Practitioner or Physician Assistant to join a busy outpatient Internal Medicine practice. You will provide primary care, population health, and immediate care services to Unite Here union members, with an office-based call schedule shared across the team and autonomy in patient management.

Qualifications

• Master's degree in Nursing required.
• Nurse Practitioner or PA credentialed in IL before hire.
• BLS within 1 month of hire.
• DEA/Controlled Substances license eligibility.

Responsibilities

• Obtain patient history and perform assessments.
• Provide direct treatment and management or referrals.
• Counsel patients and families on health options and resources.
• Document care in accordance with policy.

Malpractice coverage with tail

• Join an established, stable and well-respected large medical group practice setting
• Provide primary care, population health, and immediate care-type services to Unite Here union members (hospitality workers and their adult dependents)
• Call structure: office call, rotated equally with the team
• Practice setting: Outpatient
• Busy Internal Medicine practice
• See 10-20 patients per day on average

How you’ll make an impact in this role

• Obtains patient history and performs assessment via observation, interview and examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screening, immunizations and patient care.
• Provides direct treatment and management of health conditions via referral to other healthcare providers or community resources.
• Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources.
• Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
  • Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

• Preferred Credential(s):
• Registered Nurse.

Education

• Master's degree of Nursing required.
